Please, identify the chip on the following picture.

This chip may be an operational amplifier, but I'm not sure.
Package: SOT23-5

Pinout:
1= Output
2= Vcc (5V)
3= Input
4= Input
5= GND

According to picture and pinout that you wrote in message, this IC may be an Microchip MCP6001R. Datasheet at the attachment.

Chip replaced with the suggested one, and the machine came to life again.
